Even in the countries where the majority of the population lives in rural areas, the resources are concentrated in the cities. All countries have difficulties with transport and communication, and they all face the challenge of shortages of doctors and other health professionals in rural and remote areas. Many rural people are caught in the poverty– ill health–low productivity downward spiral, particularly in developing countries. The Global Initiative on Rural Health. This health dispensary project proposal is aimed at helping the residents in receiving proper and quality health services in the community. This is to save the locals from travelling long distances to access better health facilities. The mission of this project is to improve and construct modern health facilities comprising of all required services rendered by any modern hospital with a vision to provide dual services to the indigenous people, to take care of the victims of HIV/AIDS, to extend the service to all unreached places, and to provide timely and appropriate care to women during and after pregnancy and children under five years to reduce the maternal morbidity and infant/child mortality as well as achieving a good outcome for the baby. This sample proposal also aim at providing home based education, to promote the message that HIV/AIDS infection does not mean death is at hand, to provide spiritual education and support, and to have a nursing school in future.
EXECUTIVE SUMMARY
This health dispensary project aimed at helping the residents in receiving proper and quality health services in the community. This is to save the locals from travelling long distances to access better health facilities.
This health dispensary is a community organization owned institution run by the founding director and other members which comprises of the board members in management.
The dispensary has got two branches located in different villages with an aim of expanding the service to all unreached places, the first dispensary is located at village-1 and another one is located at village-2 which provide affordable primary health care services to a population of more than 15 villages of approximately 30,000 people in XXXX and XXXX Divisions, and also many other people come from different parts of the region. The project includes Public health supervisor, Birth attendant, Nutritionist-cum- Health Educator and supervisory technical staff providing support to field staff. The focus of the project is providing services on general health, family planning and mother, child Immunization and also the victims of HIV/AIDS.

OBJECTIVES
- To construct and finish up the health dispensary for the population
- To reduce the incidence of mortality and morbidity, malnutrition antenatal care, post- natal care and Immunization services.
- To support and supplement special health related activities and preventive programs such as literacy training for sanitation and low cost methods of providing safe drinking water and hygienic latrines.
- To provide employment to the locals and improve their living standards.

PROBLEM STATEMENT
Tanzania, like many countries of the developing world, is faced with the challenge of providing adequate health services to all her people. However, available national Resources, especially finance, are insufficient to implement this mammoth task.
Within the XXXX village there is only one government clinic located near this area but it does not address all the health needs services. The Roads in this area are not well passable thus during the night it is difficult to find Transport to access mother and child health services. As a result many expecting women deliver at home or on the way without being attended by health personnel. This causes a High incidence of child mortality.
Our current infrastructure is not large enough to meet the rising demand for medical services in both villages. We currently have a one-room clinic with only one hospital bed, where we serve 65-75 people a day. We are also faced with an insufficient drug supply in the dispensary. The dispensary lacks a modern laboratory to facilitate medical investigation for effective holistic clinical management, mortgage, X-Ray machine, Ultrasound machine, Ambulance, wards for admitting out patients, theatre for carrying operations, and a secured environment which is fenced.
