Referring to 3.8, you are applying to address a particular issue in your area because you have experience working in it. If you have that experience, you may have also drawn out some lessons. These lessons can be noted down here. If there are lessons or past experience from other partners or even other organizations, even these can be listed out here.
The Section 4 of the application form has questions related to partnerships like the added value of the applicant organization to the project: this could be your expertise, knowledge and your close relationship with your community. Partners you have listed earlier should have some kind of existing relationship with your organization. You have worked with your partners like the village development committee or the government service providers earlier. You can explain this in 4.2.
4.3 is about the details of the capacity of your partners. If they have been part of similar initiatives earlier, then their experience can be very valuable. You can mention this in the application form.
As soon as the concept note is completed, make sure you read and re-read it to finalize it. In addition to the concept note, you need to complete the logical framework, which can be downloaded from this link. A specific and official guide on how to write this logical framework in GPAF is available at this link.
A budget template is also available at this link to be filled and submitted along with the concept note and the logical framework.
