The Baxter International Foundation is accepting grant applications from organizations in the United States and around the world for programs or projects that improve the access, quality and cost-effectiveness of direct healthcare services. Major purpose of the grant is to make a positive and lasting impact on healthcare and the health of communities around the world.
Healthcare here includes dental, mental health, substance abuse, and health services to the disabled, domestic violence, sexual assault, and child abuse. The Foundation grants will address critical needs, focusing on communities where Baxter has a significant corporate presence.
Eligibility Criteria
- To be selected for funding, the proposed project must be in response to valid need and should be superior to other competing projects.
- Applicant must be able to demonstrate evidence of the organization’s capacity to accomplish its goals and continue the project post grant period.
- Preference is given to proposals that expand and increase access to healthcare with a strong plan for sustaining the services or program beyond the term of The Baxter International Foundation grant.
Requirements of the application include –
- The organization’s full legal name, complete mailing address, phone number, fax number, contact person’s name and e-mail address.
- Description of the organization’s relationship with the local Baxter facility, if applicable.
- Brief but complete description of the organization.
- Statement describing the specific purpose of the grant requested.
- Statement of the amount of money requested in U.S. dollars and time period.
- A plan for how the program or services will be sustained after the conclusion of grant funding from The Baxter International Foundation.
- A plan for measuring results and reporting periodic progress, as well as for a final evaluation.
- Financial information, including the organization’s current operating full budget and the budget for the proposed project.
- A list of sources of income and amounts, including support from corporate donors, foundations, the United Way and the government – committed and approaching – for both the organization and the proposed project.
- Audited financial statements for the most recent fiscal year and the two previous years (total of three).
- A copy of the organization’s certificate of tax exemption for the Internal Revenue Service as a 501(c)3 or equivalent organization. For organizations outside the United States, a copy of the charitable certificate and exemption from taxes from the local or national government.
- List of officers and board members, with their home addresses and their affiliations.
- List of the names and home addresses of the five highest paid employees.
