The XXXX to make the proposed program community healthcare services through mobile clinic. The implementation procedure will start on October, XXXX to March, XXXX in the support workers, resource persons and Human resources of the State. The organization fully aware about all these aspects of the program and be confident about their involvement in the program.
We feel that the donor agency will consider the project for sanctioning grant-in-aid for Healthcare program for Tribal and Non-Tribal disadvantage Villagers.
Problem Statement
Due to lack of knowledge, information and orientation in health and hygiene at the grass root level, villagers cannot understand the need of immunization, importance of growth monitoring, technique of low cost nutritious food preparation, different methods of birth control, spacing between two children, importance using sanitary or pit type latrine, preparation of safe drinking water, maintenance of personal hygiene and disposal of waste products from the home and practices to maintain good health. In fact, sound health deteriorates herewith the increase of superstition and wrong method of treatment. So, the incidence of maternal mortality, child mortality, morbidity, dehydration and malnutrition rate and other infectious diseases are quite high as per our cannot diagnosis. The existing Dai are not qualified so they cannot diagnosis in the case of high-risk pregnancies properly. The quacks are not trained. They depend on limited indigenous knowledge. The diversity and multiplicity of the problem can be decreased with some comprehensive program in this matter. At the time of feeling pain, they have to take to distant primary Health Centre but on the way the pregnant women face great problem. Sometime the pregnant women are compelled to give birth their children under the open sky. A child has been born on the road and that is why the child has been named as ‘Pathik’ (in English it is ‘street’). A child has been born in the cornfield and the child has been named as ‘Banamali’ (in English it is called ‘Bush’). When they come to the health center of Government, there is no adequate arrangement for delivery and treatment. So, most of the patients have to go to XXXXX hospital, but some of them die in the street.
XXXXXX District is the lowest illiteracy and backward place in XXXXX state in India. The district also a Malaria effected district declared by the Government of India. A serious problem exists in the rural villages of XXXXX and XXXXX districts are that the children are dying from common illness and infections that are attributable to poor nutrition. By addressing this problem many weaker sections our society can get benefit at very significant level. The weaker section of the society, particularly, Poor villagers are impacted the most.

The use of volunteers to provide community service is a new concept in Health-related program and can be capitalized upon as a viable way to provide trained manpower for the offering of educational services. The emphasis of project has been exclusively on the providing of information to communities on improved health practices. The selection and training of volunteers is conducted autonomously at village panchayat and Primary Health Centre (PHC) with the support of the instructional materials disseminated by the donor agency and other stakeholders.
Proposed Strategy
At the grass root/village level: At this level we have to formed Community Organizations as cluster of village/Mahila Samiti in XX Villages under XXXXXXX and XXXXXX District of state. The community organizer will preliminarily detect the high-risk mother and children through home and she will visit and person to person communication method and she will also conduct group discussion to identify the rural need of the community people. This grass root level organization will help us to form an institutional base and for better participation in any program implementation. The community organizer will also help the village level Dai and people to make them conscious about the importance of birth control through different methods, immunization, regular health check-up for pregnant mother and children up to 0-5 years of age, to diagnosis the high-risk pregnant mothers and children prevention of epidemic diseases, oral re-hydration therapy. They will arouse general health clubs’ members. We will also arrange for mothers meeting, baby show, immunization camp and mother and child health on a regular basis. We can ensure for strengthening our existing resources to work with Dai (Traditional Birth Attendant) and community organizer for successful implementation of the program of the grass root level.
Intermediate level: At this level Health post will be conducted by public Health Supervisor and physician (MBBS), project staff and volunteers will be providing through Mobile Clinic monthly basis at our adopted XX villages. The village people including women and children will assemble at the health post at village level. They will consult with the doctor. The doctor will make them aware through treatment, advisor, lecture, showing, children will be attended to by the Mobile unit and referral service will be rendered to relevant mother and children at our Hospital and on the basis of the severity of the patients they will be referred central level hospital. At this stage we will follow a scientific management information system for quick decision making and to take up appropriate treatment for the poorest of the poor mother and children. At this state all information will come to the physician through community organizer(s) for quick action and referred services to reduce maternal mortality, child mortality and morbidity malnutrition and re- hydration rate. This system will be followed from the inception of the program. The Mobile unit will also arrange camp time to time for better coverage and will monitor the activities of Dai and community organizers.
Central Level/ Referral service: At this level under the leadership of a Medical Officer (Physician) and Health Supervisor all the activities of the project will be monitored and corrective measure will be taken from time to time for effectiveness of the program and for better outcome to achieve our ultimate goal Health and education for all and economic self-reliance amongst women folk in our target group in XXXXXX. The high-risk mothers and children will be treated in our Hospital Campus for safe delivery and good health for children and mothers. At this stage referred services will be given to mother and children during emergency. To ensure whether we are proceeding as per plan and scheduled we will form an impact Evaluation Team. It will look into outcome of the program and its effectiveness and suggest corrective measures. An outdoor Clinic and counselling Centre for rural folk at the Central level will be organized from the central community Health Centre. Outdoor and indoor patients will be treated from this health Centre.
In the proposed Community Healthcare Centre period 2 Months will be cover survey of the project first two months. Poor villagers will be secure good result from this proposed Centre. When addressing a result frame work and determining the information logic, the borrower identifies the project to solve by the target group within project period. Indicators should be specific, measurable, attributable, realistic and Time bound (SMART) indicators. And 94% had final achievements, 58% had targets, 6% had baseline.
