The goal of the Comprehensive Support Services and Case Management component is to offer participants in the Supportive Housing Programme who are homeless a comprehensive support network. This element seeks to meet the participants’ many requirements, including those related to their bodily and mental health, employment, education, and social integration.
By offering comprehensive case management, individualized support plans, and access to a network of service providers, the program seeks to empower participants on their journey towards stability, self-sufficiency, and community integration.

- Initial Assessment and Individualized Support Plan:
- Upon enrollment in the program, each participant will undergo a comprehensive assessment to identify their specific needs, strengths, and goals.
- Based on the assessment, a personalized support plan will be developed in collaboration with the participant, outlining the steps and resources needed to achieve self-sufficiency and stability.
- Healthcare and Mental Health Services:
