It is recommended that you follow the instruction of each form and fill up regular information including personal and NGO information. For the form SF-424, the Name of Federal Agency is: “The United States for Agency for International Development (USAID)”, the Catalog of Federal Domestic Assistance Number/Title is: “Saving Lives at Birth: A Grand Challenge for Development (Round 3)”, and the Funding Opportunity Number/Title is: “RFA-OAA-13-000004/Discretionary”.
For the form SF-424A, it is a budget form requested more details compared to the budget explanation in Section III, and it only requests the budget to be filled as unit forms but not too many details. A budget with all of the details and its narratives will not be needed until if you pass the innovation screening and are requested, which will be described in later section. What we need to keep in mind is that all of the budgets you fill in have to match exactly what you write in the activities, monitor and evaluation sections. You need to break down the total budget into smaller units which include activities and other categories according to the form and form filling instruction, and fill all of the available units in the form accordingly. The differences between the direct cost and indirect cost are that the direct cost is those cost directly for finishing the project like personnel, consumables, and other necessary costs; and the indirect cost is those cost requested by the space and facility provider like your employer or your space provider specifically for this project. In addition, funding sources other than the USAID funding, and if mentioned in Section III, are also required to fill in this form according to the time frame of each quarter, but not in itemized details.
As an assurance form of the SF-424B, you need to read through all of the items and then sign and date at the end of the form to show that you fully agree with the assurances.
