Executive Summary
Non-communicable diseases (NCDs), including cardiovascular diseases, diabetes, cancer, and chronic respiratory illnesses, account for over 70% of global deaths each year. A significant proportion of these deaths occur in low- and middle-income countries, where health systems are already overburdened and preventive care remains weak. Many NCDs are preventable through early awareness, lifestyle changes, and community-level interventions.
The project “Preventing Non-Communicable Diseases Through Community Awareness” aims to reduce the burden of NCDs by empowering communities with knowledge, skills, and supportive environments that promote healthy behaviors. The initiative focuses on increasing awareness of NCD risk factors, encouraging early screening, and strengthening community engagement in preventive health practices.
Implemented over 24 months, the project will work closely with community members, local health workers, schools, and grassroots organizations. Through education campaigns, behavior change communication, capacity building, and linkages to primary healthcare services, the project will promote sustainable lifestyle changes. The initiative aligns with SDG 3 (Good Health and Well-Being) and supports national and global NCD prevention strategies.
Problem Statement
Non-communicable diseases pose a growing public health challenge worldwide. Rapid urbanization, changing dietary patterns, sedentary lifestyles, tobacco and alcohol use, and increased stress levels have significantly contributed to the rising prevalence of NCDs. Despite their long-term impact, NCDs often remain undetected until complications arise, leading to increased healthcare costs, productivity losses, and premature mortality.
In underserved communities, limited awareness and misinformation about NCDs exacerbate the problem. Many individuals are unaware of early symptoms, risk factors, or the importance of regular screening. Preventive healthcare services are often underutilized due to lack of information, cultural beliefs, financial constraints, and limited access to health facilities.
Women, low-income households, elderly individuals, and informal workers are particularly vulnerable. Poor nutrition, physical inactivity, and untreated conditions such as hypertension and diabetes contribute to intergenerational cycles of poor health and poverty.
While healthcare systems primarily focus on curative care, prevention and community-based awareness programs receive limited attention and funding. There is a critical need for integrated, community-driven approaches that promote healthy behaviors, improve health literacy, and strengthen links between communities and primary healthcare services.
Target Population
- Primary Beneficiaries
- Adults at risk of NCDs (ages 18–60)
- Women and elderly individuals
- Low-income and marginalized households
- Youth vulnerable to unhealthy lifestyle habits

Implementation Plan
The project will be implemented over 24 months in three phases. The first phase will focus on assessment, planning, and community mobilization. The second phase will implement awareness activities, capacity building, and screening support. The final phase will strengthen community ownership, evaluate outcomes, and document best practices for replication.
A project coordinator, community mobilizers, and trained volunteers will ensure effective implementation in collaboration with local health authorities.
