Executive Summary
Strong and resilient health systems are essential for achieving equitable health outcomes, responding to public health emergencies, and ensuring long-term social and economic development. However, many health systems, particularly in low- and middle-income and underserved regions, face persistent challenges such as workforce shortages, weak institutional capacity, limited use of data and technology, fragmented service delivery, and inadequate preparedness for emerging health threats.
The Strengthening Health Systems Through Capacity Building and Innovation project is a comprehensive multi-year initiative designed to enhance the performance, resilience, and sustainability of health systems by investing in human resources, institutional strengthening, and innovative solutions. The project will focus on building the skills of healthcare workers and managers, strengthening health institutions, improving service delivery models, and integrating digital and data-driven innovations to improve efficiency and quality of care.
Through targeted training, mentorship, system-level reforms, and pilot innovations, the project aims to improve access to quality healthcare services, strengthen health governance, and enhance the ability of health systems to respond to current and future health challenges. The initiative emphasizes local ownership, equity, and sustainability to ensure lasting impact beyond the project period.
Problem Statement
Health systems across the world are under increasing pressure due to population growth, epidemiological transitions, aging populations, and the rising burden of non-communicable diseases, alongside persistent infectious diseases. These pressures are compounded by limited financial resources, inadequate infrastructure, workforce shortages, and weak management and governance structures. In many underserved settings, health facilities struggle to deliver essential services consistently and equitably.
A critical challenge is the limited capacity of the health workforce. Healthcare workers often lack access to continuous professional development, modern clinical practices, leadership training, and digital skills. Health managers may have limited experience in strategic planning, data-driven decision-making, financial management, and quality improvement. This capacity gap directly affects service quality, patient safety, and system efficiency.
In addition, many health systems have not fully leveraged innovation and technology to improve service delivery. Weak health information systems, limited interoperability, poor data quality, and low adoption of digital tools hinder effective planning, monitoring, and accountability. Innovation initiatives, when introduced, are often fragmented and not integrated into broader system reforms.
Without strategic investments in capacity building and innovation, health systems will continue to struggle to meet population needs, respond to emergencies, and achieve universal health coverage. Strengthening health systems is therefore a critical priority for sustainable development and improved population health outcomes.
Target Beneficiaries
The primary beneficiaries of this project include:
- Healthcare workers, including doctors, nurses, midwives, and allied health professionals
- Health facility managers and administrators
- Public health officials and health system planners
- Community health workers and frontline service providers
Secondary beneficiaries include patients and community members who will benefit from improved quality, accessibility, and continuity of healthcare services. Health institutions, including hospitals, clinics, and public health agencies, will also benefit from stronger systems, improved governance, and enhanced operational efficiency.
Goal and Objectives
Overall Goal:
To strengthen health systems by enhancing capacity, improving governance, and promoting innovation to deliver equitable, high-quality, and resilient healthcare services.

- Knowledge sharing and learning platforms
- Monitoring, evaluation, and adaptive learning
Project Activities
- Health Workforce Capacity Building
The project will deliver targeted training programs for healthcare workers focusing on clinical best practices, patient-centered care, infection prevention, quality improvement, and emerging health priorities. Training will be delivered through blended learning approaches, combining in-person workshops, e-learning modules, and practical simulations. - Leadership and Management Development
Health managers and administrators will receive training in leadership, strategic planning, financial management, supply chain management, and performance management. Coaching and mentorship will support the application of skills in real-world settings. - Institutional Strengthening
Technical assistance will be provided to strengthen governance structures, standard operating procedures, referral systems, and coordination mechanisms across levels of care. Emphasis will be placed on improving accountability, transparency, and service integration. - Digital Health and Innovation Pilots
The project will support the design and implementation of digital health solutions, such as electronic health records, telemedicine platforms, decision-support tools, and mobile health applications. Pilot innovations will be evaluated for effectiveness and scalability. - Data Systems and Use for Decision-Making
Capacity building will focus on improving data collection, quality, analysis, and use. Health information systems will be strengthened to support monitoring, evaluation, and evidence-based planning. - Knowledge Sharing and Learning
Communities of practice and learning platforms will be established to facilitate knowledge exchange among health professionals, managers, and policymakers. Lessons learned will be documented and shared to inform broader health system reforms.
Implementation Plan
The project will be implemented over a three-year period in three phases. The first phase will focus on assessments, stakeholder engagement, and detailed planning. The second phase will involve capacity building, system strengthening, and innovation pilots. The final phase will emphasize consolidation, scale-up of successful innovations, policy engagement, and sustainability planning.
A dedicated project management unit will oversee implementation, working closely with health authorities and partners to ensure alignment and coordination.
Monitoring and Evaluation
A comprehensive monitoring and evaluation framework will track progress, outcomes, and impact. Baseline, midline, and endline assessments will measure changes in workforce capacity, service quality, system efficiency, and patient outcomes.
